Donate 5 per thousand to Emergency

How do
- Fill in the form of the 730 or CUD.
- Signature in the box marked "Supporting non-profit social organizations, associations of social promotion ..."

Even those who must submit a tax return can still apply for the card to your employer or institution paying your pension and give it (completed and in a sealed envelope) to a post office, at a bank, who receive free or a qualified intermediary to transmit electronically (CAF, accountants, etc..). The envelope must write DESTINATION FOR FIVE THOUSAND PIT and indicate full name and social security number of the taxpayer.
